okay…So you meant that specific setting is NOT applicable when different users send a single message at the same time.

For that specific setting, with the assumption that they all choose the setting to have the messages forwarded to their emails. What if 100 different users send separate messages to different recipients at the same time (meaning that one sender sends a message to one recipient and total 100 messages), will all the messages be forwarded to the emails simultaneously? or it will take 45 mins for 100 messages to be forwarded?

One question: every time I upgrade the plugin, do I have to change the file permission? I thought I changed it the last time. Now, it appears that those file permission change was gone for some reason.

Yes, all the fields are filled at the front end settings > emails.
I went over and checked all 6 points shown in the link one by one. They are all fine at my site. I also checked the junk mail of the recipient. I tested the SMTP and piping email which is <relay@mydomain.com>. It is also fine.
Still, the recipient does not receive email when he receives new message.

Is it possible to include */staging and */clone since those are the patterns given by some web-server hosting providers?
It will be great if you would consider including them in your list. As of now, I cannot test your plugin in those environments.

I saw the following at your link. Does * mean “mydomain.com” in this case?
For instance, staging.* means staging.mydomain.com?
If that is the case, the followings are not going to be in the categories of mydomain.com/staging or mydomain.com/clone
Does it mean I cannot apply the license to my staging/clone environments? Would you be able to increase those into */staging or */clone or */test etc?

License keys for wordpress plugin sold by this website can also be activated in local environments without increasing your site count.
But your website URL must matches one of these:
localhost
*.dev
*.local
dev.*
